What to Know About:
Wellworks Wellness

Reminder, we are in the Plan Year for our Wellworks Wellness Program that was introduced last year.

Why are we participating in this plan?

To build healthy preventive healthcare habits by going to a medical provider and having basic testing done on an annual basis. We believe this will help our employees with early identification and treatment on any potential medical issues.

Who is required to participate?

Employees & Spouses (if enrolled) on the medical plan on or before 9/1/25. Children do not need to participate.

What is the program?

The Program consists of:

  • Annual Physical
  • Annual Blood-work
  • Annual Tobacco Attestation
    If a tobacco user, Wellworks includes an opportunity for a 6-week Tobacco Cessation Program.

How do I get credit for participation?

Log into the Wellworks website via www.wellworksforyou.com

You can also use the Wellworks Mobile App via, Wellworks For You Mobile App

  • Login details can be found on Dayforce under
    • Benefits
      • Wellworks Wellness Plan
        • Click the link to the “Wellworks Portal Log In Instructions”

Go to your Wellness Locker in Wellworks

  • Upload your completed form
  • Complete the Tobacco Attestation Form (specifics for what is considered Tobacco are included on the form)

How does the program impact new employees after 9/1/2025 through 8/31/2026?

  • This group of new employees will receive the Wellness Rate in 2027.
  • They will need to complete the requirements during the program year (9/1/26-8/31/27) to qualify for the 2028 Wellness Rate.
  • This timeline repeats ongoing, on an annual basis.

What is the impact in 2027 to the Wellness Rates?

  • The rates remain the same for full participation.
  • Rates for Partial or Non-participation are specific to the plan you are enrolled in and are outlined on Page 2 of the Wellworks Program Guide.

Is the $50.00 incentive payment for having my physical exam still available?

Yes, through 8/31/26 your can still upload your annual physical exam information into Dayforce. Each insured employee and insured spouse can receive one $50.00 payment per 1/1/26 through 8/31/26.

This is the final year of the $50.00 incentive. Starting 9/1/26 the incentive will be reflected in the Wellness Rates.

Please refer to the Wellworks Wellness Program on your Dayforce Hub page for Links to set up/access your account, upload your forms, QR Codes and other important information to assist you.
