Wellness Program

Important Reminder!!
The August 31, 2026 deadline for participation in the Wellworks Wellness Program is fast approaching!
Why is this so important?
Paycheck deductions will likely increase on 1/1/27 for enrolled Employees. To be eligible for the lower monthly deductions (“Wellness Rate”), you are required to complete the Wellness Program steps.
Why is OHFL participating in this plan?
We believe this will help our Employees with early identification and treatment of any potential medical issues.
Ongoing participation will also build healthy preventive healthcare habits by going to a medical provider and having basic testing done on an annual basis.
Who is required to participate?
Enrolled Employees & Spouses (if enrolled) on our medical plan
on or before 9/1/25.
Children on plans do not need to participate.
What is the program?
The Program consists of:
- Annual Physical
- Annual Blood-work
- Annual Tobacco Attestation
If you are a tobacco user, there is an opportunity for a 6-week Tobacco Cessation Program.
*NOTE: Our medical plans allow for an annual physical and blood-work at no cost to Employees (if using an in network provider). Co-pays and Deductibles are waived.
When do I need to complete the program requirements?
The program runs annually from 9/1 through 8/31 of the following year. Therefore, the current program year is 9/1/25 through 8/31/2026.
How do I get credit for participation?
Log into the Wellworks website via www.wellworksforyou.com
You can also use the Wellworks Mobile App via, Wellworks For You Mobile App
Go to your Wellness Locker in Wellworks
- Upload your completed form
- Complete the Tobacco Attestation Form (specifics for what is considered Tobacco are included on the form)
How does the program impact new employees after 9/1/2025 through 8/31/2026?
- This group of new employees will receive the Wellness Rate in 2027.
- They will need to complete the requirements during the program year (9/1/26-8/31/27) to qualify for the 2028 Wellness Rate.
- This timeline repeats ongoing, on an annual basis.
Is the $50.00 incentive payment for having my physical exam still available?
Yes, through 8/31/26 your can still upload your annual physical exam information into Dayforce. Each enrolled employee and enrolled spouse can receive one $50.00 payment, from 1/1/26 through 8/31/26.
This is the final year of the $50.00 incentive.
